Introdução: O aumento na demanda pelas cirurgias plásticas pós-bariátricas evidenciou a alta prevalência de psicopatologias nessa população, exigindo a necessidade do diagnóstico dessas doenças no pré-operatório. A utilização de ferramentas para triagem psicológica específica, já na primeira consulta, tem sido estimulada pela maioria dos autores, entretanto, um método padrão-ouro ainda não foi plenamente estabelecido e a busca persiste. Objetivo: Realizar uma revisão da literatura sobre as alternativas disponíveis para a avaliação psicológica preliminar de pacientes candidatos a cirurgias plásticas pós-bariátricas, apresentando a conduta preconizada no ambulatório de Cirurgia Plástica Pós-Bariátrica da Universidade Federal de Mato Grosso do Sul (UFMS). Métodos: Revisão dos ensaios clínicos, que empregaram ferramentas de triagem psicológica pré-operatória em pacientes candidatos à cirurgia plástica pósbariátrica, nos bancos de dados MEDLINE/PubMed, utilizando os descritores "bariatric surgery", "body image", "quality of life", "obesity", "plastic surgery" e "psychiatry", nos últimos 20 anos, discutindo os achados e analisando as metodologias mais utilizadas. Resultados: Foram encontrados apenas 4 ensaios clínicos que utilizaram ferramentas de triagem psicopatológica no pré-operatório de cirurgias plásticas pós-bariátricas, sendo que um método de eleição não pôde ser identificado. Conclusão: A utilização de estratégias apropriadas para a triagem das psicopatologias auxilia na prevenção de prejuízos significativos no pós-operatório, porém a construção da ferramenta ideal ainda carece de validação na população pós-bariátrica. Além do amplo conhecimento técnico clínicocirúrgico, o cirurgião plástico deve manter-se atento aos sinais e sintomas psicopatológicos desses pacientes, encaminhandoos para avaliação psiquiátrica e psicológica quando indicado.


Introduction: The increase in demand for post-bariatric plastic surgery has revealed a high prevalence of psychopathologies in patients undergoing the procedure, requiring the need to diagnose these diseases in the preoperative period. The use of specific psychological screening tools has been promoted; however, a gold-standard method has not yet been fully established. Objective: To carry out a review of the literature for alternatives available for the preliminary psychological evaluation of patients who are candidates for post-bariatric plastic surgery, presenting the method recommended in the Post-Bariatric Plastic Surgery outpatient clinic of the Federal University of Mato Grosso do Sul (UFMS). Methods: We reviewed the clinical trials which employed psychological screening tools preoperatively in patients who were candidates for post-bariatric plastic surgery; the MEDLINE/PubMed database was searched using keywords such as "bariatric surgery", "body image", "quality of life", "obesity", "plastic surgery" and "psychiatry", for clinical trials published in the last 20 years. Herein, we discuss the findings and analyze the most common methodologies used. Results: Only four clinical trials used psychopathology screening tools in postbariatric plastic surgeries preoperatively, and one method could not be identified. Conclusion: The use of appropriate strategies to screen for psychopathologies helps prevent significant losses in the postoperative period, but the tools still lack validation in the post-bariatric population. Besides possessing extensive clinical-surgical technical knowledge, the plastic surgeon must remain attentive to the signs and psychopathological symptoms in these patients, referring them for psychological and psychiatric evaluation when indicated.

Generally, the main complications of silicone implantation are local symptoms. However, some patients develop late-onset systemic symptoms often associated with a rare form of hyperactive immune response, as part of a syndrome known as autoimmune syndrome induced by adjuvants (ASIA). Reported cases of ASIA have shown resolution with explantation, but not with immunomodulatory therapy. In this report, we described a case of a previously healthy 23-year-old woman, who has undergone silicone breast implant augmentation, for aesthetic reasons, and developed localized cutaneous impairment 3 years postsurgery. She received a diagnosis of ASIA with a new presentation: Lupus-like manifestation through localized cutaneous impairment. This patient's symptoms were managed without the need for surgical intervention, which has not been previously reported, because the patient did not want an explantation for aesthetic reasons. The patient was started on hydroxychloroquine, 400 mg per day, and remains asymptomatic after 2 years of treatment. The exact predisposition to ASIA is still unknown. Without implant explantation and with immunomodulatory treatment, this patient's condition substantially improved. Based on our current understanding of this disease, it might not be prudent to indicate breast augmentation with silicone implants in patients with documented autoimmune reaction to an adjuvant, an established autoimmune condition, or genetic predisposition. However, if a patient does develop silicone-induced ASIA, explantation is no longer the only successfully reported option, as these symptoms can be managed with immune suppression.

PURPOSE: To determine the effects of end-to-side nerve repair performed only with fibrin glue containing nerve growth in rats. METHODS: Seventy two Wistar rats were divided into six equal groups: group A was not submitted to nerve section; group B was submitted to nerve fibular section only. The others groups had the nerve fibular sectioned and then repaired in the lateral surface of an intact tibial nerve, with different procedures: group C: ETS with sutures; group D: ETS with sutures and NGF; group E: ETS with FG only; group F: ETS with FG containing NGF. The motor function was accompanied and the tibial muscle mass, the number and diameter of muscular fibers and regenerated axons were measured. RESULTS: All the analyzed variables did not show any differences among the four operated groups (p>0.05), which were statistically superior to group B (p<0.05), but inferior to group A (p>0.05). CONCLUSION: The end-to-side nerve repair presented the same recovery pattern, independent from the repair used, showing that the addition of nerve growth factor in fibrin glue was not enough for the results potentiating.


OBJETIVO: Determinar os efeitos do reparo nervoso término-lateral realizado apenas com cola de fibrina contendo fator de crescimento nervoso em ratos. MÉTODOS: Setenta e dois ratos Wistar foram distribuídos em seis grupos: A - não submetido à secção nervosa; B - secção do nervo fibular (sem reparo); Os outros grupos tiveram o nervo fibular seccionado e então reparado na superfície lateral do nervo tibial intacto, com diferentes procedimentos: C - RNTL com suturas; D - RNTL com suturas e FCN; E - RNTL apenas com CF; F - RNTL com CF contendo FCN. A função motora foi acompanhada e a massa do músculo tibial, o número e o diâmetro das fibras musculares e axônios regenerados foram medidos. RESULTADOS: Não houve diferença entre as variáveis avaliadas nos quatro grupos operados (p>0,05), os quais foram superiores ao grupo B (p<0,05), mas inferiores ao grupo A (p>0,05). CONCLUSÕES: O reparo nervoso término-lateral mostrou o mesmo padrão de recuperação, independente do tipo de reparo utilizado, evidenciando que a adição de fator de crescimento nervoso na cola de fibrina não foi suficiente para a potencialização dos resultados.

OBJECTIVE: Morphological study of effects of silver sulfadiazine, Tabebuia avellanedae (ipê-roxo) extract and Stryphnodendron adstringens (barbatimão) extract on cutaneous wound healing was done. METHODS: Ninety six Wistar rats were used. All animals underwent a femoral right vein ligation to induce a venous hypertension. Thirty days after the cutaneous wound was done, they were divided into groups of four animals. Group S received silver sulfadiazine topical application; group IR, Tabebuia avellanedae extract topical application; group B received Stryphnodendron adstringens extract topical application and group C received physiological solution topical application, every day for a period of seven, 14 and 30 days. Histology analyzed the presence vascular proliferation, neutrophil and lymphocite, fibroblast, collagen fiber and epithelization. RESULTS: The macroscopic analysis showed complete epithelization at 14 days in group S, IR e B. The histological data at 14 days of observation only group C still showed incomplete epithelization in six animals. At the same period there was a statistic significant difference between the control group and the others groups as inflammation process and neovascularization. About the presence fibroblasts and collagen, there was statistic significant difference between the control group and the others groups at the 30 days because at this period the control group still showed fibroblasts and collagen lower when compared to the others groups. CONCLUSION: The morphological analysis of the results permits to infer that the group S, IR and B had a better healing of skin wounds, when compared with the control.

OBJETIVO: Estudo morfológico do efeito da sulfadiazina de prata, extrato de ipê-roxo e extrato de barbatimão na cicatrização de feridas cutâneas. MÉTODOS: Utilizou-se 96 ratos Wistar. Todos foram submetidos à ligadura da veia femoral direita para produzir hipertensão venosa. Após 30 dias foi confeccionada a ferida cutânea. Dividiu-se os animais em quatro grupos. O grupo S recebeu aplicação tópica de sulfadiazina de prata; o grupo IR, extrato de ipê-roxo; o grupo B, extrato de barbatimão e o grupo C, aplicação de solução salina a 0,9 por cento, diariamente, nas feridas por um período de sete, 14 e 30 dias. A análise histológica avaliou: proliferação vascular, neutrófilos, linfócitos, fibroblastos, fibras colágenas e epitelização. RESULTADOS: Os achados macroscópicos mostraram epitelização completa aos 14 dias em todos os animais dos grupos S, IR e B. Na análise histológica aos 14 dias, apenas o grupo C ainda apresentava epitelização incompleta em seis animais; neste mesmo período houve diferença estatisticamente significativa entre o grupo controle e os demais grupos quanto ao processo inflamatório e neovascularização. Em relação à presença de fibroblastos e colágeno, houve diferença estatisticamente significativa entre o grupo controle e os demais grupos aos 30 dias. CONCLUSÃO: A análise dos resultados morfológicos permite inferir que o grupo S, IR e B foram favorecidos no processo de cicatrização das feridas cutâneas, quando comparados com o controle.
